12-Month In-Person Care | Greenwich, CT
This is The Elm’s most comprehensive service: a year-long program designed for women who want more than a one-time visit. With Emilee Bischof, WHNP-BC, as your dedicated clinician, you’ll receive ongoing care that blends gynecology, hormone therapy (HT), lifestyle medicine, and weight management (when appropriate) into one seamless plan.
Our approach is proactive, personalized, and designed to evolve with you — because women’s health in midlife requires more than quick visits and one-size-fits-all solutions.
Who Is This Program For?
This program is designed for women who want a dedicated partner in their health — not just a quick appointment. It’s ideal if you’re in midlife and looking for in-person care in Greenwich, CT that goes beyond the basics.
With this program, you’ll have:
- In-person access to a dedicated clinician for comprehensive exams and ongoing support
- Personalized hormone therapy (HT) and preventive care tailored to your stage of life
- Continuous guidance on everything from weight changes to sleep, mood, and long-term wellbeing
How This Program Elevates Your Care
At The Elm, we understand it can feel natural to look for a single appointment to “fix” what’s going on. But women’s health — especially in midlife — is rarely that simple. Quick visits don’t allow enough time to explore the full picture of your symptoms, review your history, or create a safe plan that truly works long term.
That’s why our care is structured as a 12-month program. It gives you the time, continuity, and access you deserve — so we can move beyond short-term fixes and build lasting health together.

Plenty of time for thoughtful conversation, in-depth review of your health, and detailed planning tailored to you.
Gynecology + Preventive Care

Rather than masking symptoms, we take time to understand your history, lifestyle, and goals to address the full picture of your health.

Most women in perimenopause or menopause experiencing symptoms such as hot flashes, low libido, sleep disruption, brain fog, fatigue, mood changes, or weight gain are strong candidates. The best way to know for certain is to schedule an initial consultation so we can review your health history and goals.
-

Age alone does not rule you out. Our clinicians will review your cardiovascular and overall health profile to assess safety. Many women benefit from hormone therapy well beyond the “ideal window.”
